JULY 30TH 3-5PM

SUMMER REGISTRATION

MONDAY-FRIDAY- 9AM-2PM

DOCUMENTATION NEEDED/REQUIRED

Proof of residency (deed or lease for an address in Baltimore County-Glyndon ES Zone)

Three documents that contain a date within 60 days, the parent / guardian's name, and address under which student is being enrolled

Photo identification of parent/guardian enrolling student

Birth certificate or other documentation of student's name and age

Proof of immunization (Students missing required vaccine(s) must have an appointment to obtain missing vaccine(s) or records within 20 days)

School Registration Form
Health History Form

Transfer papers from prior school (Grades 1 - 12)

IEP/504/Student Support Plan (If applicable)

Prior Care Form (Kindergarten Only)

Proof of income & complete income eligibility form (Preschool / Prek Only)
